Not sure I follow you exactly. Are you using the auto option for adding images to the gallery? If so, the plugin only picks up the first image attached to the post, ie subsequent images attached to the post won't override the first one.
Does that make sense?

I have the same issue. I added an image to a post. It did not look right in auto mode. So I deleted image and added another one. However it still trying to show the first one I added. How do I delete this first image from DCG picked for a post. I see in the MYSQL database DCG Optoins does not remove or update image is selected for a post when I delete and add a new one. How do I reset this auto selected images in the database and make DCG find the first images in all posts again.

"Auto" works by grabbing the first image attachment in the post. So, you need to unattach the first image, then upload a new image and all should be fine. Alternatively, use the "Full" image menagement option and enter whatever URL you want in the DCG Metabox.

Also having this exact same problem, and it's annoying :)
I am using "auto".
1) I create a new post with text, then I set my featured image, then I publish the post. DCG is working properly and looks great…
2) The problem happens if I decide I want a NEW picture: I will edit the post, remove the current featured image, add a NEW featured image, update post. DCG is still showing the OLD/DELETED photo (but Wordpress shows the NEW image).
UPDATE: As a test, I repeated step 1… but in step 2 I ONLY deleted the photo & didn't add a new one. Well, both DCG & Wordpress show the old/deleted photo! So even though there isn't an image associated with that post anymore, it's still messing up.
I like the auto feature, is there a way to RESET/CLEAR the original photo?
UPDATE – FIXED: The solution is to edit the post, next to the "upload/insert" click the "add an image" icon, once the "add an image" window pops up, click on "gallery" tab, and delete the offending image!
To delete it just click on "show", then "delete" at the bottom… problem solved!

OfflineIndeed, that's exactly how to deal with this. Thanks for posting!
For other readers: "Auto" grabs the FIRST image attachment. Therefore, if you upload a second image to the post and want to use this as the image in the DCG, you must delete the first attachment. A pain? Yes. However, the next version of the DCG will exclusively use Featured Images, which means that the selected Featured image will appear in the gallery. :-)

This probably stems from me not fully understanding how DCG and Wordpress work. I was having the same problem and even the solutions here didn't help at first because of the way I was expecting/understanding things to work. This is just a quick explanation to help anyone who has my mindset.
When using the auto method the first image attachment is not the first image you see in the post or page, as I thought. As JP mentioned above it is the first image in the post's/page's gallery. You get to the gallery using the method JP explains in his "UPDATE – FIXED" paragraph above.
The only problem is when you delete the image from the gallery it completely removes it from the media library. Not great when you use the same image on other pages as well. I have seen several posts in the Wordpress codex asking for this to be changed. I did find a plug-in which helped me detach images from a gallery without removing it from the media library. Wordpress Media Library Plug-in.
There is another catch as well. I added a lot of images I am going to use in bulk via the media tab. I then inserted them into posts and pages using the Media Library tab on the Insert/Add pop-up. This doesn't add the image to the gallery and hence it doesn't appear in DCG as I assumed it would.
I am looking forward to the featured image being used. When will that be available (I have 3.3.5 installed)? You mentioned using the featured image exclusively, does that mean the DCG image URL field will go away? I don't know if this is my misunderstanding of how Wordpress works again but it would be nice to be able to set an image the size of the gallery in DCG image and have smaller image in the featured image that Wordpress would use. Am I misunderstanding things again?
I love the plug-in, especially now as I am starting to understand exactly how it works.

OfflineThanks for your useful post, Darren. Image handling in WP is not that intuitive and your advice will help others. 
Development on v4 (which uses Featured Images) is near completion but keeps being delayed due to irritating things like earning a living… My current deadline is September.
